髋部骨折患者的肌少症相关危险因素分析 |

中文摘要:【】 目的:骨骼肌减少症是以随着年龄增加的骨骼肌质量减少及骨骼肌力量减少,功能下降为的一种病症,简称肌少症。此项研究的目的在于探讨肌少症在髋部骨折患者中的发生率及相关风险因素。方法:本研究搜集我院在2013年5月至2017年1月于我院诊治的髋部骨折病人199例,男109例,女 90例,年龄20-95岁,平均年龄74.82±16.64岁。根据观察指标,收集患者的一般情况,四肢骨骼肌肉容积(ASMI),脂肪总量,骨矿含量(BMC),身体质量指数(BMI),握力,术前血维生素D,血白蛋白,美国麻醉医师协会分级评分(ASA)及NMS运动评分。根据握力和ASMI,将患者并分为肌少症组与非肌少症组,采用单因素及多因素分析统计方法。结果:此研究病人中符合肌少症诊断的病人99例(49.7 %),与非肌少症组相比,肌少症组与高龄,高ASA,低脂肪总量,低骨矿含量,低BMI,低维生素D,低白蛋白,低NMS评分相关,随后进行的二元logistic回归分析提示高龄OR=1.087(p=0.011),高ASA评分 OR=2.912( p=0.002),低脂肪总量OR=0.854(p= 0.012),低骨矿物质盐OR=0.214(p=0.035)。结论:高龄,高ASA评分,低脂肪总量,低骨矿物质盐可能是髋部骨折病人肌少症的相关危险因素 |

ABSTRACT 【】 Objective: Skeletal muscle atrophy is a kind of illness with reduced skeletal muscle mass and decreased skeletal muscle strength with age. The purpose of this study was to investigate the incidence of sarcopenia in hip fracture patients and related risk factors. METHODS: In this study, 199 patients with hip fractures diagnosed and treated in our hospital from May 2013 to January 2017 were recruited. There were 109 males and 90 females aged 20-95 years with an average age of 74.82±16.64 years. According to the observations, the general conditions of the patients were collected, appendicular skeletal muscle index (ASMI), total fat mass, bone mineral content(BMC),body mass index(BMI), grip strength, preoperative blood vitamin D(VITD), albumin, American Society of Anesthesiologists physical status classification(ASA)and new mobility scores(NMS). According to grip strength and ASMI, the patients were divided into sarcopina group and the non-sarcopina group, and univariate and multivariate statistical methods were used. Results: In this study, 99 patients (49.7%) were diagnosed with sarcopenia. Compared with the non-sarcopenia group, sarcopenia was associated with advanced age, high ASA, low total fat mass, low bone mineral content, low BMI, low VITD, low albumin, and low NMS were correlated. Subsequent binary logistic regression analysis suggested that older patients had OR 1.087 (p=0.011), high ASA score OR=2.912 (p=0.002), and low total fat OR = 0.854 (p = 0.012), low bone mineral salt OR = 0.214 (p = 0.035). CONCLUSIONS: Old age, high ASA score, low fat mass, low bone mineral content may be associated risk factors for sarcopenia in hip fracture patients. |
