| 中文摘要:摘要：目的 通过对照研究的方法探讨切开复位内固定治疗肱骨近端骨折中采用肱二头肌长头腱切断再固定的临床疗效。方法 自2014年1月至2017年1月应用切开复位内固定治疗肱骨近端骨折的患者50例，其中男28例，女22例，年龄范围69～92岁，平均74.0岁。根据术中是否采用肱二头肌长头腱切断再固定分为对照组、治疗组各25例。比较两组术后1周疼痛评分、术后1、3、6、12月肩关节Neer功能评分、骨折愈合情况及术后并发症。结果 50例患者均获得随访，随访时间12～24月，平均14.5个月。术后1周疼痛评分、术后1、3月肩关节Neer功能评分有明显差异（P<0.05），术后并发症、术后6月、1年肩关节Neer功能评分未见明显差异（P>0.05）。结论 肱二头肌长头腱切断再固定对予肱骨近端骨折术后功能恢复具有一定的作用，有利于降低术后疼痛，加快肩关节功能早期恢复，值得临床进一步推广。 |

| Clinical comparative study of long head of biceps tendon cutted-off and re-fixation for the treatment of proximal humeral fractures |

| ABSTRACT Abstract：Objective To study the clinical effect of open reduction and internal fixation in the treatment of proximal humeral fracture by using long head of biceps tendon cutted-off and re-fixation. Methods From January 2014 to January 2017, 50 cases of proximal humeral fractures were treated with open reduction and internal fixation, including 28 males and 22 females. The age range is 69~92 years, with an average age of 74 years. According to whether the long head of biceps tendon cutted-off and re-fixation, the patients were divided into the control group and the treatment group, 25 cases in each group. The clinical data of two groups were retrospectively analyzed. Results All 50 patients were followed up for 12-24 (14.5) months. Between the two groups in postoperative 1 week pain score and after 1, 3 months shoulder joint Neer function scores were obviously different（P<0.05）. But postoperative complications and after 6, 12 months shoulder joint Neer function scores were no significantly different（P>0.05）. Conclusion Long head of biceps tendon cutted-off and re-fixation combined with open reduction and internal fixation has a certain effect on postoperative functional recovery of senile patients with surgical neck fracture of humerus. This method can reduce postoperative pain and speed up shoulder joint function recovery, which is worthy of further promotion. |
