基于动态捕捉的坐位腰椎旋转手法的运动力学及相关影响因素

中文摘要:目的:研究坐位腰椎旋转手法操作的运动力学参数并进行初步分析验证,与此同时探讨该手法操作的影响因素以提高临床应用价值。方法:自2016年1月至2016年12月在中国中医科学院望京医院门诊就诊退行性腰椎滑脱患者30例,其中男18例,女12例;年龄45~61(52.33±2.34)岁;病程2~72(29.13±3.23)个月。1)应用运动捕捉技术,测量同一手法操作者向30例患者施者行坐位腰椎旋转手法的运动力学参数(旋扳时间、最大速度、最大加速度)。2)对比术者左右手参数的不同。3)探讨并分析不同患者所具备的个体特征对坐位腰椎旋转手法操作所带来的影响。结果:1)坐位腰椎旋转手法运动力学参数平均值如下:旋扳时间0.64±0.03s,最大速度517.56±33.24 mm/s,最大加速度3264.51±127.55mm/s2。2)左右手施行的坐位腰椎旋转手法的力学参数差异无统计学意义(p>0.05)。3)经多元线性回归分析,患者的年龄对操作者的旋扳时间相关具有显著性(P<0.01);患者的身高对操作者的最大速度与最大加速度有显著的相关性(P<0.01)。结论:1)左右手施行的坐位腰椎旋转手法有着相近的力学参数,因此左右手的手法操作在临床应用上并无较大区别。2)经过研究发现,在坐位腰椎旋转手法操作过程中,受试者的年龄、身高等均是重要的影响因素。

Kinematic mechanics and related factors study of seated lumbar rotation manipulation based on dynamic capture
ABSTRACT  Objective: In this observational trial, the kinematic and mechanical parameters of seated lumbar rotation manipulation are recorded, analyzed and verified preliminarily. At the same time, the influence factors of the manipulation are discussed to enhance the clinical significance of this manipulation. Methods: From January 2016 to December 2016, 30 patients with degenerative lumbar spondylolisthesis were treated in Wangjing Hospital, Chinese Academy of Chinese Medicine, including 18 males and 12 females, aged from 45 to 61 (52.33±2.34) years, with a course of 2 to 72 (29.13±3.23) months. 1)They had been operated the seated lumbar rotation manipulation by the same manipulator. During the manipulation, the kinematic and mechanical parameters of the 30 patients were dynamically traced by the body mechanics measurement instrument, including the spin time, the maximum speed and the maximum acceleration. 2)The differences in the parameters of the right and left hands of the operator were compared. 3)The influence factors of individual physical characteristics on the manipulation were analyzed. Results: 1)The kinematic and mechanical parameters of manipulation were as follows: spin time was 0.64±0.03s, maximum speed was 517.56±33.24mm/s, maximum acceleration was 3264.51±127.55mm/s2. 2)The manipulation done by right or left hand showed the similar parameters (P > 0.05). 3)According to multivariate linear regression, there was a significant correlation between the age of the patient and the rotation time of the operator (P < 0.01); the height of patient had a significant correlation with the maximum speed and maximum acceleration of the operator (P < 0.01). Conclusions: 1)There is no difference between the manipulation of left and right hand in clinical application. 2)Age and height are important influencing factors of seated lumbar rotation manipulation.
