目的:探讨应用一种新的带血供植骨方法治疗股骨头缺血坏死的疗效。
方法:采用头颈开窗,死骨清除,带血供磨菇状植骨治疗股骨头缺血坏死患者31例。按Ficat分期:ⅡA期10例,ⅡB期13例,Ⅲ期8例。旋髂深骨瓣移植13例,缝匠肌骨瓣5例,阔筋膜张肌骨瓣4例,大转子骨瓣9例。
结果:随访1.5～4年,根据王岩等的诊断标准综合评价:优20例,良7例,可2例,差2例,优良率为87.1%。
结论:头颈开窗磨菇状植骨可以改善和重建股骨头的血供,避免股骨头应力集中,预防和治疗股骨头塌陷。 |

| Treatment of avascular necrosis of femoral head with pedicled muchroom bone graft through fenestration on the femoral head and neck |

Abstract:
Objective:To explore the curative effects on avascular necrosis of femoral head(ANF) by a new method of pedicled muchroom bone graft through fenestration over the femoral head and neck
Methods:According to Ficat classification,there were 10 caes of stage ⅡA of ANF,13 of stage ⅡB and 8 of stage Ⅲ.The vascular pedicled bone graft were applied in 13 cases,ilioc bone flap with sartorius muscle in 5 cases,tensor fasciae latae pedicled iliac bone graft in 4 cases and greater trochanter bone flap in 9 cases.31 cases of avascular necrosis of femoral head were treated with pedicled mushroom bone graft through fenestrations on the femoral head and neck
Results:The average follow up period was 1.5 to 4 years.The excellent results were found in 20 cases,good in 7 cases,fair in 2 cases,poor in 2 cases and excellent rate was 87.1%based on Wangyan′s standard.
Conclusion:Mushroom bone graft could improve and reconstruct the blood circulation of femoral head,avoid stress concentration prevent and treat the collapse of the femoral head. |
