目的：探讨下肢骨折合并血管损伤的治疗方案对预后的影响。
方法：45例下肢骨折合并血管损伤的病人采取石膏托外固定5例，髓内针内固定5例，钢板内固定14例，外固定架固定18例，加压螺纹钉3例；血管损伤单纯修补13例，端口吻合19例，大隐静脉移植15例。
结果：随访平均3.5年，肢体成活率为57.7%（26/45）,总截肢率40%（为18/45）,死亡率为2.3%（1/45）,
结论：肢体的预后主要取决于完全缺血时间、骨折程度、血管损伤程度以及软组织破坏程度。而正确处置损伤血管及骨折固定方式是肢体成活的关键。 |

| Analysis of Treatment on Lower Limb’s Fracture complicated with Vascular Injury |

ABSTRACT
Objective:The influence of treatment scheme of lower limb’s fracture complicated with vascular injury on its prognosis was explored.
Methods: Out of 45 patients with lower limb’s fracture complicated with vascular injury,5 cases were fixed externally with gypsum,5 cases with intramedullary nail,14 cases with internal plates,18 cases with external fixator,and 3 cases with compressed screws.The injured vessels were treated with simple repair in 13 cases,with endtoend anastomosis in 19 cases,and with grafting of great saphenous vein in 15 cases.
Results: After following up for 3.5 years in average,the survival rate of limbs was 57.7%(26/45),the total amputation rate 40%(18/45),and the mortality 2.3%(1/45)
Conclusion: The prognosis of injured limbs was mainly determined by complete ischemic time,and the extent of fracture,vascular injury and soft tissue injury.The key to survival of limbs was the correct treatment for limb fracture and vascular injury. |
