目的:研究应用中西医结合方法治疗创伤性骨髓炎合并骨缺损的疗效。
方法:对132例创伤性骨髓炎合并骨缺损的患者根据创面细菌培养和药敏试验结果,分别选用有效中药于自控中药湿热敷治疗机上恒温冲洗创面2～4周,配合敏感抗菌素的应用,行病灶彻底清除和复合组织移植Ⅰ期治疗。
结果:132例中2例手术失败,126例获得1～6年随访,121例治愈,2例骨髓炎复发,3例骨不愈合。
结论:在中西医结合控制感染基础上,应用显微外科技术Ⅰ期修复创伤性骨髓炎合并骨缺损,可以提高疗效,缩短病程,减少费用,是一种切实可行的有效途径。 |

| Integration of TCM and western medicine for the treatment of traumatic osteomyelitis combined with osseous defect |

ABSTRACT
Objective: To study clinical results of integration methods of TCM and western medicine for the treatment of traumatic osteomyelitis combined with osseous defect.
Methods:Bacterial culture and drug Sensitive tests were done with material procured in the wound in 132 cases of osteomyelitis combined with osseous defects;all the patients were treated with integration methods of TCM and western medicine:(1)The wound was amply lavaged with solutions made from traditional Chinese drugs(TCD) under constant temperature condition,twice daily and each time for 30min for 2～4weeks.Self controlled therapeutic machine for TCD wet heat dressing was used in this treatment.(2) Application of sensitive antibiotics;(3) Radical removal of the focus and (4) one stage composite tissue grafting.
Results:Of the 132 cases,2 were surgical failures;among the 126 cases followed up for 1～6 years,121 were cured.Osteomyelitis recurrences in 2 cases and osseous nonunion occurred in 3.
Conclusion: This method could improve the therapeutic effect,shorten the course of illness;it is a cost effective method for the treatment of traumatic osteomyelitis. |
