髋臼横断骨折不同内固定方法对臼顶负重面积的影响

目的:采用压敏片技术测量髋臼横断骨折及采用不同内固定方法后臼顶负重区的负重面积。

方法:取成年男性防腐尸体骨盆5具(含双侧股骨中上段)。标本分为如下各组:Ⅰ组(完整骨盆),A组(前柱单钢板内固定),B组(后柱单钢板内固定),C组(前柱单钢板加后柱单钢板内固定),D组(前柱拉力螺钉加后柱单钢板内固定),用特制的夹具将骨盆和股骨固定于单足站立中立位,去除髋关节囊,将压敏片贴于股骨头上,分别测量各组标本髋臼臼顶负重区的负重面积,对所得数据通过SPSS100软件进行方差齐性检验,LSD法多重比较。

结果:Ⅰ组,A组,B组,C组,D组的臼顶负重区负重面积(cm2)分别为:443±057、236±042、261±044、330±052、34±041。对以上5组中臼顶负重面积进行LSD法多重比较,结果为Ⅰ组与A、B、C、D四组差别均有显著性意义(PIA<001,PIB<001,PIC<005,PID<005),A组与B组差别无显著性意义(PAB>005),C组与D组差别无显著性意义(PCD>005),A、B两组与C、D两组间差别均有显著性意义(PCD>005)。

结论:对于髋臼横断骨折,即使给予解剖复位内固定,髋臼臼顶负重面积也不能恢复至正常。

Influence on the loading area in the dome region of transverse acetabular fracture with different fixations
ABSTRACT  

Objective: To investigate the changes on the loading area in the dome region before and after different transversal acetabular fracture with internal fixation methods.

Methods:Five embalmed pelves plus the proximal femur were harvested for this study.Before loading,the entire capsule of the hip joint was removed.The pressure sensitive films were fixed on the surface of the femoral head with 502 glue.Test was initially performed with the intact acetabulum.After the intact pattern(Group Ⅰ) was recorded,sequential internal fixation methods were performed.Group A:anterior column plate;Group B:posterior column plate;Group C:anterior column plate and posterior column plate;Group D:anterior column lag screw and posterior column plate.To these five groups,the loading area in the acetabular dome region were measured with pressure sensitive film then multiple comparisons among five groups were performed with LSD method.The difference was significant at the 0 05 level.The statistical work was performed with the help of software SPSS 10 0.

Results: For the Group Ⅰ(Intact),Group A(Plate/None),Group B(None/Plate),Group C(Plate/Plate),Group D(Screw/Plate),the loading area were as follows(cm 2):4 43±0 57,2 36±0 42,2 61±0 44,3 30±0 52,3 34±0 41.Through LSD multiple comparison,there was significant difference between Group I and Group A,B,C,D (PIA<0 0 1,PIB<0 0 1,PIC<0 0 5 ,PID<0 0 5 ).There was no significant difference between Group A and B (PAB>0 0 5 ).There was no significant difference between Group C and D(PCD>0 0 5 ).There was significant difference between Group A,B and Group C,D((PCD>0 0 5 ).

Conclusion: For the transversal acetabular fractures,the loading area in the dome region did not recover to normal even after anatomical internal fixation.From the view of biomechanics,double column fixation methods were better than single column fixation methods on the raised loading area.Double column fixation methods were ideal methods for transversal acetabular fractures.
