目的:评价弧轨自锁弓根螺钉内固定系统(ALPF)治疗胸腰椎骨折脱位的临床效果。
方法:胸腰椎骨折脱位387例,男239例,年龄19～71岁,平均(45.6+13.8)岁;女148例,年龄18～65岁,平均(45.6±8.2)岁。采用ALPF技术治疗,并对其进行疗效分析。
结果:367例获得随访,随访时间6～64个月。患者神经功能、脊柱活动、腰背痛和下肢痛的改善率分别为80.4%、65.9%、92.1%和87.4%,术前术后椎体前、后缘高度恢复均明显(P<0.01,P<0.05),后凸角得到满意矫正(P<0.01)。并发症43例,其中术中并发症12例,发生率为3.1%;术后并发症31例,发生率为8.4%。
结论：ALPF椎弓根固定技术治疗胸腰椎骨折脱位疗效确定,并发症少。 |

| Analysis of the effect of arc-track self-locking pedicle fixation system(ALPF)in thoracolumbar spinal fracture-dislocation |

ABSTRACT
Objective:To evaluate the clinical results of arc-track self-locking pedicle fixation system(ALPF)for the treatment of thoraolumbar spinal fracture-dislocation.
Methods:From May 1997 to January 2003,387 patients of thoracolumbar spinal fracture-dislocation underwent ALPF (male,239 cases;female,148 cases).The average age of the male was(45.6+13.8) years (range from 19 to 71 years).The average age of the female was (45.6±8.2)years (range from 19 to 71 years).The results were evaluated.
Results:Three hundreds and sixty-seven patients were followed up for 6～64 months.The amelioration of patients' nerve function,spinal motion,low back pain and leg pain was 80.4%,65.9%,92.1% and 87.4% respectively.The height of anterior and posterior border after operation were improved greatly than that before operation(P<0.01,P<0.05).The X-ray films showed that the angle of kyphosis was also improved significantly(P<0.01).Complications occurred in 43 patients,among whom 12 cases occurred during operation with the incidence of 3.1%,while 31 cases occurred after operation with the incidence of 8.4%.
Conclusion:The clinical results revealed that ALPF for the treatment of thoracolumbar spinal fracture-dislocation is satisfactory,with few complications. |
