目的:评价多束血管束植入软骨面开窗植骨治疗成人股骨头坏死的疗效,探讨其手术适应证及术中术后注意事项。
方法:各种病因所致Ⅱ～Ⅲ期成人股骨头坏死患者30例,男28例,女2例;年龄28～45岁,平均37.8岁。采用经股骨头前外侧软骨面开窗病灶清除植骨,利用旋股外侧动静脉的升支、横支等组成多条血管束,经骨隧道植入股骨头治疗。术后牵引6～8周,患肢半年内不负重活动。
结果:30例患者平均随访5.4年,根据成人股骨头坏死疗效评价法,评定手术前后髋关节疼痛、功能、关节活动度及X线表现,总分比较差异有显著性(P<0.01)。
结论：股骨头软骨面开窗病灶清除植骨并多束血管植入治疗Ⅱ～Ⅲ期成人股骨头坏死疗效满意,开窗软骨瓣术后都能得到修复,手术操作简单,股骨头内病灶可直观彻底清除,利于新骨形成。 |

| Treatment of femoral head necrosis by cartilage fenestration with vascular bundle implantation |

ABSTRACT
Objective: To evaluate the effect of cartilage fenestration with vascular bundle implantation on treatment of femoral head necrosis,and to examine operational adaptability and matters for attention.
Methods:Thirty patients of femoral head necrosis of diverse pathogeny(male 28 cases,female 2 cases;Age from 28 to 45 years,with average of 37.8 years) were treated by cartilage fenestration at femoral head to remover focus and plant bone,then implant vascular bundles composed of branches of artery and vein via marrow passage,and by 6～8 weeks of traction and half year without load.
Results:Followed-up with average of 5.4 years.According to the evaluation of femoral head necrosis in adult,pre-and postoperative evaluation of hip joint pain,function,flexibility and X-ray performance showed significant difference(P<0.01),with excellence rate 76.6% and efficacy rate 90%.
Conclusion:Treatment of femoral head necrosis by cartilage fenestration with bone and vascularized implantation show good results,fenestrated cartilage could be repaired easily,and focus within femoral head could be removed completely,all of them help growth of new bone. |
