目的:探讨中青年股骨颈骨折的临床治疗效果。
方法:自1996年5月-2002年6月收治中青年股骨颈骨折102例(GardenⅠ、Ⅱ型40例,Ⅲ、Ⅳ型62例),采用Richards钉内固定(A组)26例,AO多枚加压空心螺纹钉内固定(B组)32例,股方肌骨瓣转位移植加多枚加压空心螺钉内固定(C组)44例。
结果:所有病例经平均39个月的随访,三组病例骨折愈合时间分别平均为192、145、94d,C组和B、A组差异有显著性意义(P<0.05,P<0.01);骨折愈合率平均为73%,91%,100%,C组和B、A组差异有显著性意义(P<0.05,P<0.01);功能优良率分别为50%,75%,95%,C组和B、A组差异有显著性意义(P<0.05,P<0.01);髋内翻发生率、股骨头坏死率分别为15.04%,12.56%,6.81%和23.08%,15.63%,6.81%,三组间差异无显著性意义(P>0.05)。
结论:股方肌骨瓣转位移植加多枚加压空心螺钉内固定具有愈合时间短,骨折愈合率和功能优良率高等优点,是治疗中青年股骨颈骨折较理想的方法。 |

| Clinical observation of femoral neck fracture in middle-age with three different methods |

ABSTRACT
Objective:To investigate the clinical effect of femoral neck fracture in middle-age.
Methods:Form May 1996 to June 2002,102 patients (40 cases were Garden Ⅰ?Ⅱ type and 62 cases Garden Ⅲ?Ⅳ type)were treated with internal fixation of Richards screw (group A,26 cases),AO compression hollow screw (group B,32 cases),translocation graft of quadratus femoris and AO compression hollow screw (group C,44 cases).
Results:All the patients were followed up with an average of 39 months.In three groups,the healing time of fracture were 192,145,94 days.The rate of healing were 73%,91%,100%.The rate of functional excellent and good were 50%,75%,95%.The incidence rate of coxa adducta were 15.04%,12.56%,6.81%.The rate of femoral head necrosis were 23.08%,15.63%,6.81%,respectively.All the data were analyzed with statistics.There were significant differencs in the healing time of fracture,the rate of healing and the rate of functional excellent and good among the three groups (P<0.05,or P<0.01).There were no significant difference in the others aspect(P>0.05).
Conclusion:Translocation graft of quadratus femoris with internal fixation with AO compression hollow screw is the better method for the treatment of femoral neck fracture in middle-age,which have the advantages of short-healing time,high healing rate and high rate of functional excellent and good etc. |
