目的:分析以尺骨鹰嘴牵引为主治疗肱骨外科颈、骨干、髁上、髁间骨折的方法和疗效。
方法:收治病例共66例,男46例,女20例;年龄8-78岁,平均38岁。外科颈骨折21例,骨干骨折8例,伸直型髁上骨折32例,髁间骨折5例。所有骨折均为新鲜骨折。上述骨折多数在行牵引前有1-2次手法整复失败史。本组均采用尺骨鹰嘴牵引为主,配合闭合复位、小夹板加纸压垫外固定治疗。
结果:所有病例随访6-24个月,依照《中医病证诊断疗效标准,》66例中治愈61例,好转2例,未愈1例,另有2例肱骨干骨折中途因软组织嵌插而改行切开复位内固定。功能恢复满意达90%。所有小儿肱骨髁上骨折均治愈,无肘内翻发生。
结论:应用尺骨鹰嘴牵引为主的方法治疗肱骨外科颈、骨干、髁上、髁间骨折能使骨折较好复位,促进软组织损伤的修复,关节功能恢复好,肘内翻的发生率低。 |

| Skeletal traction of olecranon combined with closed reduction in the treatment of the humeral fracture |

ABSTRACT
Objective:To study the therapeutic effects of skeletal traction of olecranon as major therapy in the treatment of humeral fractures including surgical neck,humeral shaft,supracondylar and intercondylar fracture.
Methods:Sixty-six patients with fresh humeral fractures,included 46 male and 20 female,with an average age of 38 years ranging from 8 to 78 years,were recruited in the present study.Among the patients,21 patients were surgical neck of humerus fracture,8 patients were humeral shaft fracture,32 patients were humerus-straightened type of supracondylar fracture,and 5 patients were intercondylar fracture.Most patients had one or two times of unseccessful treatment with manipulation before admission.In the present study,all the patients were treated with skeletal traction of olecranon combined with closed reduction and small splint external fixation.
Results:After a followed-up period of 6 to 24 months,61 patients gained excellent result,2 good and 1 bad according to the criteria of Diagnosis and Therapeutic Effect of Orthopaedic and Traumatologic Diseases and Syndromes in Traditional Chinese Medicine.Two patients with humeral shaft fracture were treated with the open reduction and internal fixation because of soft tissue wedge.The rate of excellent and good therapeutic effect was 90%.All children′s supracondylar fractures healed completely without cubitus varus.
Conclusion:In the treatment of humeral fracture skeletal traction of olecranon is effective to gain fracture reduction,improve soft tissue repair,recover joint function and decrease the occurrence of the cubitus varus. |
