目的:观察脊柱(定点)旋转复位法治疗巨大腰椎间盘突出症临床疗效并探讨其机制。
方法:采用脊柱(定点)旋转复位法治疗巨大腰椎间盘突出症29例,男20例,女9例;年龄14～57岁,平均33.8岁。按患者主观症状、临床体征、有无运动受限进行临床评分,比较手法治疗前后评分结果并进行统计学分析。测量患者椎间盘MRI轴位像突出髓核大小,计算反映突出相对比率的矢状径指数SI值,比较手法治疗前后SI值并进行统计学分析。
结果:手法治疗后患者主观症状、临床体征、运动受限得分明显增加(P<0.05),治疗前后临床综合评分分别为(10.8966±3.6873)、(20.0345±2.7449),治疗后较治疗前显著提高(P<0.05)。手法治疗前后SI值分别为(0.6288±0.1069)、(0.5689±0.1478),统计学检验无显著性改变(P>0.05)。
结论:腰椎间盘巨大突出并不意味严重的神经损伤,临床表现受多因素影响。脊柱(定点)旋转复位法可治疗巨大腰椎间盘突出症,其机制为纠正患椎椎体位移,恢复脊柱内外因素平衡。 |

| Treatment of lumbar disk herniation with a huge herniated mass by FENG Tian-you spinal manipulation |

ABSTRACT
Objective:To evaluate the clinical effect of treatment of lumbar disk hernation with a huge herniated mass with FENG Tian-you spinal manipulation.
Methods:Twenty-nine patients including 20 male,9 female with the average age of 33.8 years old,ranging from 14 to 57 years,were treated by FENG Tian-you spinal manipulation.The clinical results were measured by the Japanese and FENG Tian-you lumbar treatment standard and evaluated by scoring system for assessing the treatment of low back pain before and after treatment.MRI sagittal index(SI)of lumbar disk hernation was measured as well before and after treatment.
Results:The scoring of patients’ subjective symptoms and clinical signs were improved obviously(P<0.05).The treatment score was(10.896 6±3.687 3) and(20.034 5±2.744 9)before and after treatment respectively,the score was increased significantly after treatment(P<0.05).The SI between pre-and post-treatment was(0.628 8±0.106 9) and(0.568 9±0.147 8),with no statistical difference(P>0.05).
Conclusion:The clinical manifestation of lumbar disk herniation with a huge herniated mass is affected by many factors,a huge herniated mass does not mean nerve injuried severely.FENG Tian-you spinal manipulation plays an important role in the treatment of lumbar disk herniation with a huge herniated mass by correcting the single or multiple vertebral subluxation and restoring the inner-outer balance of spinal column system. |
