中文摘要:
目的:介绍一种对前交叉韧带(ACL)胫骨附丽处撕脱骨折的微创治疗途径。
方法:ACL胫骨附丽处撕脱骨折患者81例,男64例,女17例;年龄10～58岁,平均37.8岁;交通伤57例,坠落伤15例,运动伤6例,其他伤3例。按Zaricznyj分类:Ⅰ型3例,Ⅱ型16例,ⅢA型21例,ⅢB型25例,Ⅳ型16例。利用经髌下纵切口插入膝关节腔内的食指导引,予ACL胫骨附丽处撕脱骨折复位后,经皮空心钉内固定;骨折块小、Ⅳ型骨折或骨骺未闭合时,钢丝固定。
结果:对81例患者进行6～24个月随访,平均18个月,术后3个月膝关节活动度与健侧相比无明显区别,骨折全部愈合,Lysholm膝关节功能评分为(91.6±3.2);术后6个月评分为(95.7±3.6)。
结论:ACL胫骨附丽处撕脱骨折髌下微创治疗,操作简便、微创,效果可靠。 |

| Minimal invasive treatment of anterior cruciate ligament avulsion fracture from the tibia spine |
|
ABSTRACT
Objective:To introduce a minimal invasive surgical procedure for the treatment of anterior cruiate ligament(ACL) avulsion fracture of the tibial attachment.
Methods:Eighty-one patients with ACL avulsion fracture of the tibial attachment were treated involving 64 males and 17 females with an average age of 37.8 years(10-58 years).The transportation wound were in 57 cases,the crash wound in 15,the movement wound in 6,other wound in 3.According to Zaricznyj classification,3 cases were of typeⅠ,16 of type Ⅱ,46 of type Ⅲ(21 of ⅢA,25 of ⅢB),16 of type Ⅳ.Adopted the longitudinal incision of the knee,the fracture was fixed by cannulated screw after the avulsion fractures was diaplasis following the conduction of forefinger or fixed by steel wire if the bone was small or type Ⅳ fracture or the epiphysis was closed.
Results:All of the fractures had a good union compared with healthy knee 3 months after operation in an average follow-up for 18 months(6-24 months).The mean Lysholom knee score was(91.6±3.2),and(95.7±3.6) six months after operation.
Conclusion:Advantages of this method are minimal invasive,simple technique,and can lead to reliable reduction and fixation. |
