| 中文摘要:【摘要】腰椎间盘突出症作为骨科的常见病、多发病,目前临床采取的治疗手段主要包括保守治疗及手术治疗。经皮椎间孔镜下髓核摘除术(percutaneous transforaminal endoscopic discectomy,PTED)与传统的开放手术相比,具有安全、有效、经济、微创等优点,目前被广泛应用于腰椎间盘突出症的微创治疗。但临床上报道的腰椎间盘突出症在经皮椎间孔镜椎间盘切除术治疗后失败的病例也屡见不鲜。根据国内外相关学者的研究报道,导致手术失败的危险因素多种多样,目前尚无统一定论,主要包括术前患者、适应症、手术方式、麻醉方式的选择,术中并发症的发生及术者的操作技术,术后感染、遗留痛及合理的康复锻炼等相关因素。本文通过对部分失败PTED的文献进行广泛回顾,就目前临床上失败PTED的发生概况、导致失败的危险因素、以及防治对策等方面做一综述。 |

| Overview of risk factors for failed percutaneous transforaminal endoscopic discectomy in lumbar disc herniation |

| ABSTRACT The lumbar disc herniation is a common and recurrent disease in the Department of orthopedics. At present, the treatment means mainly include conservative treatment and surgical treatment. Compared with traditional open surgery, percutaneous transforaminal endoscopic discectomy (PTED) is safe, effective, economical and minimally invasive. It is widely used in minimally invasive treatment of lumbar disc herniation. However, the clinical reports of the failure of PTED are also common.According to the research reports of domestic and foreign scholars, there are varieties of risk factors for surgical failures,including the selections of patients, indications, surgical approaches and anesthesiamethods preoperative. Occurrences of surgical complicationsincluding infection or left pains, and reasonable rehabilitation exercise after the operation are related tofailures. There is no unified conclusion at present.In this paper, we reviewed the literaturesabout failed PTED, and try to make an overview about the general situation of failed operation in clinical practice, the risk factors for failures and the countermeasures. |
