桥接系统治疗成人锁骨中段骨折临床应用初探

中文摘要:【摘要】目的: 探讨桥接系统治疗成人锁骨中段骨折的临床疗效。方法:自2016年1月至2016年8月采用切开复位桥接系统内固定治疗28例成人锁骨中段骨折患者,其中男15例,女13例;年龄27~82(48.50±15.34)岁;病程13~15(14.17±0.77)个月,记录患者骨折愈合时间及并发症情况,并对术前,术后1、12个月及13个月的肩关节Constant评分进行疗效评价。结果:28例患者骨折均达到临床愈合标准,愈合时间2.5~4(3.05±0.44)个月,术后未出现感染、骨不连、延迟愈合、内固定物断裂及内固定取出后再骨折病例。28例患者获得随访,时间13~15个月(14.17±0.77)个月。术后1、3和13个月患侧肩关节Constant评分分别为76.57±4.70、90.75±3.62、96.07±2.40,与术前58.36±4.98比较差异有统计学意义 (P<0.05)。结论:桥接系统可以作为治疗成人锁骨中段骨折的一种新型内固定方式,具有恢复快、并发症少的特点,可以降低内固定物断裂、应力下骨质疏松、内固定取出后再骨折的风险。 关键词 桥接系统;锁骨中段骨折;圆棒;模块

Clinical application of bridge-link type combined fixation system to adult clavicle fractures
ABSTRACT  ABSTRACT Objective:This article discusses the effect of Bridge-link type combined fixation system in treating displaced mid-shaft clavicle fractures and provides theoretical reference for the application of Bridge-link type combined fixation system technology. Methods: From January 2016 to August 2016, 28 cases of displaced mid-shaft clavicle fractures were treated by Bridge-link type combined fixation system. A total of 15 males and 13 females, aging from 27 ~ 82 years old with an average of 48.50±15.34 years were enrolled in this study. The duration of the disease was 13 to 15 months, with an average of 14.17±0.77 months. The patient''s fracture healing time , shoulder joint Constant function score of the preoperative,1 month ,1 year postoperative and 1 month after removing of the internal fixation were graded. Results: All the patients were followed up for 13 to 15 months with an average of 14.17±0.77 months. 28 patients achieved clinical healing of fracture criteria, counterpoint good and on line, fracture healing time was from 2.5 to 4 months with an average of 3.05±0.44 months. All patients have removed the internal fixation, there were no cases of infection, bone nonunion, delayed healing, breakage of internal fixation and re-fracture after removing of the internal fixation. Shoulder joint Constant function scores of the preoperative,1 month ,3 months,13 months after operation were 58.36±4.98, 76.57±4.70, 90.75±3.62, 96.07±2.40, respectively , with significant differences compared with the scores of the preoperative(P<0.05). Conclusions: Bridge-link type combined fixation system can be used as a new internal fixation for the displaced mid-shaft clavicle fractures, it has the characteristics of rapid recovery, fewer complications and can reduce the incidence of breakage of internal fixation, osteoporosis, re-fractures after removing the internal fixation.
