| 中文摘要:目的 探讨手部动力支具配合舒筋洗外用颗粒治疗掌指关节侧副韧带挛缩的临床疗效。方法 自2017年6月至2019年1月收治掌指关节侧副韧带挛缩患者50例，按随机数字表法分为试验组和对照组各25例，分别予手部动力支具配合舒筋洗外用颗粒治疗及掌指关节松动训练治疗。其中试验组男17例，女8例，年龄平均（40.53±9.42）岁，患指共36指，病程平均（52.37±11.2）d；对照组男15例，女10例，年龄平均（42.11±8.36）岁，患指共34指，病程平均（48.24±12.5）d。观察患指肿胀、屈伸功能等情况，比较治疗前后患指肿胀情况，采用疼痛评分（VAS）评价疼痛程度，并采用关节总主动活动度（TAM）评价患指掌指关节功能恢复情况。结果 所有患者在8周疗程内均获得随访；治疗后两组肿胀程度、VAS、TAM均较治疗前改善，差异有统计学意义（P<0.05）；治疗后试验组肿胀程度、VAS、TAM较对照组明显改善，差异有统计学意义（P<0.01）。结论 应用手部动力支具配合舒筋洗外用颗粒治疗掌指关节侧副韧带挛缩，不仅可以消肿止痛而且能明显改善掌指关节活动功能，为行之有效的保守治疗方法之一。 |

| Clinical Efficacy of Power Device and Shujinxi External Granule in the Treatment of Collateral Ligament Contracture of the Metacarpophalangeal Joint |

| ABSTRACT Objective To investigate the clinical efficacy of hand power device and Shujinxi external granule in the treatment of collateral ligament contracture of the metacarpophalangeal(MCP) joint.Methods Fifty patients with collateral collateral ligament contracture of the MCP joint from June 2017 to January 2019 were randomly divided into experimental group(EG) and control group(CG) according to the random number table method,25 cases respectively.The EG were treated with hand power device and Shujinxi external granule, as the CG was given MCP joint loosening training.There were 17 males and 8 females in the EG,with an average age of (40.53±9.42) years old, 36 fingers with average disease duration of (52.37±11.2) d;15 males and 10 females in the CG, with an average age of (42.11±8.36) years old,a total of 34 fingers with the average disease duration of (48.24±12.5) d. The swelling and function of the affected fingers were observed. The swelling of the affected fingers was compared before and after treatment. The degree of pain was assessed using the visual analogue scale(VAS), and the total active range of motion(TAM) was used to evaluate the recovery of the function of MCP joint.Results All patients were followed up within the prescribed course(8 weeks) of treatment. The degree of swelling,VAS and TAM of the two groups after treatment were improved compared with those before treatment, and the difference was significant (P<0.05).After treatment, the degree of swelling,VAS and TAM of the EG were significantly improved compared with the CG,and the difference was statistically significant (P<0.01).Conclusion The use of hand power device and Shujinxi external granule in the treatment of collateral ligament contracture of the MCP joint can not only reduce swelling and pain, but also significantly improve the function of the MCP joint. It is one of the effective conservative treatment methods. |
