中文摘要:【摘要】 目的:讨论运用骨科手术机器人辅助置入骶髂贯穿螺钉的精准性及临床疗效。方法:回顾性分析自2022年8月至2023年8月行机器人辅助置入骶髂贯穿螺钉的患者24例。男性10名,女性14名;年龄21~73(49.29±14.48)岁。按Tile骨盆分型,B型13例,C型11例。根据术后CT扫描结果依据Gras标准评估螺钉置入效果。末次随访根据Matta评分评估骨折愈合情况,根据Majeed评分评估功能恢复情况。结果:总计置入骶髂贯穿螺钉36枚,S1贯穿螺钉18枚,S2贯穿螺钉18枚,共计优29枚,良7枚,差0枚。螺钉调整次数0.25±0.43次。术后随访时间3~13(6.00±3.28)个月。Matta评分优18例,良5例,中1例,最大位移距离0~12.70(3.27±3.57)mm。Majeed评分总分(84.37±8.38)分、疼痛(27.08±3.26)分、工作情况(17.00±2.70)分、坐位时疼痛(8.08±1.10)分、性生活(2.95±0.85)分、站立情况(29.25±4.24)分,其中优15例,良7例,中2例。结论:机器人能够精准、安全的辅助完成骶髂关节螺钉置入治疗骨盆后环骨折,并能促使患者术后功能恢复。 |

Accuracy and Clinical efficacy study of robot-assisted transiliac-transsacral screw placement for posterior pelvic ring fractures |

ABSTRACT Abstract OBJECTIVE To discuss the accuracy and Clinical efficacy of robot-assisted placement of transiliac-transsacral screws in the treatment of posterior pelvic ring fractures. METHODS Twenty-four patients who underwent robot-assisted placement of transiliac-transsacral screws from September 2022 to September 2023 were retrospectively analyzed. There were 10 males and 14 females; ages ranged from 21 to 73 (49.29±14.48) years. According to Tile pelvic staging, there were 13 cases of B type and 11 cases of C type. Screw placement was evaluated according to postoperative CT scan results based on Gras criteria. Fracture healing was assessed at final follow-up based on the Matta score and functional recovery was assessed based on the Majeed score.RESULTS A total of 36 transiliac-transsacral screws, 18 S1 penetrating screws, and 18 S2 penetrating screws were placed, totaling 29 excellent, 7 good, and 0 poor.The number of screw adjustments was 0.25±0.43.The postoperative follow-up period ranged from 3 to 13 (6.00±3.28) months.Matta score was excellent in 18 cases, good in 5 cases and fair in 1 case with maximum displacement distance of 0-12.70 (3.27±3.57) mm.Majeed score was total (84.37±8.38), pain (27.08±3.26), working condition (17.00±2.70), pain while sitting (8.08±1.10), sex life ( 2.95±0.85), standing situation (29.25±4.24), of which 15 cases were excellent, 7 cases were good and 2 cases were fair.CONCLUSION The robot can accurately and safely assist in accomplishing transiliac-transsacral screw placement for posterior pelvic ring fractures,and can contribute to the patient's postoperative functional recovery. |
