目的:探讨4种术式治疗先天性髋关节脱位(CDH)疗效。
方法:随访采用翻转造盖术24例(30髋)、插入造盖术23例(30髋)、Salter截骨术25例(30髋)和Chiari骨盆内移截骨术24例(27髋)治疗CDH患者96例117髋。
结果 平均随访时间5年10个月,疗效从临床功能和X线形态两方面评估,术后髋臼指数(AI)的改善分别是(20.2±7.27)°、(21.9±6.33)°、(15.0±4.56)°、(11.4±5.1)°,术后臼头指数(AHI)分别是(0.92±0.24)°、(1.00±0.18)°、(0.80±0.19)°、(0.82±0.14)°;四种术式临床优良率分别是90%、86.7%、73.4%和66.6%,共发生再脱位3例,关节僵硬10例,股骨头坏死7例。
结论:临床疗效及对髋臼形态改善翻转、插入造盖术优于Salter截骨术和Chiari骨盆内移截骨术;翻转造盖术、插入造盖术安全有效是先天性髋关节脱位的首选术式。 |

| Comparison of different surgical treatment for congenital dislocation of the hip |

ABSTRACT
Objective:To compare the clinical Results of four surgical Methods for congenital dislocation of the hip(CDH).
Methods:117 hips in 96 patients with CDH,who had the reversing shelf operation(30 hips),inserting shelf operation(30 hips),Salter osteotomy(30 hips)and Chiari Pelvic osteotomy(27 hips),were followed up with an average of 5.8 years.
Results:The radiographic and clinical Results were evaluated.The excellent and good rate of four groups were 90%,86.7%,73.4%,66.6% respectively.The improvement of acetabular morphology were investigated through measuring acetabular index(AI) mainly.AI decreased with an average of 20.2 degrees in the group of reversing shelf operation,compared with 15 degrees in Salter osteotomy.An anverage improvement of AI was 21.9 degress in the inserting shelf operation group.But it was 11.4 degrees in the Chiari pelvic osteotomy group.
Conclusion:The Results showed that the reversing shelf operation and inserting shelf operation are superior to Salter osteotomy and Chiari Pelvic osteotomy and the both operations are safe,effective and simple procedures for the treatment of congenital dislocation of the hip. |
