目的:评价应用掌侧T形锁定加压接骨板(T-locking-compression plate,T-LCP)治疗老年桡骨远端骨折的临床疗效,探讨老年骨质疏松症桡骨远端骨折的手术必要性。
方法:自2003年1月-2006年3月,应用T形锁定加压接骨板掌侧入路治疗老年桡骨远端骨折23例,骨折类型均为AO分型的B、C型;男7例,女16例,年龄60~78岁,平均66岁。术后进行腕关节功能随访,X线片观察。
结果:21例获得随访,随访时间6~18个月,平均9个月。X线示骨折全部愈合。桡骨远端关节面掌倾角平均为4.5°,尺倾角为19.2°。桡骨茎突较尺骨茎突长0.85cm。按关节功能评估标准进行评分:优11例,良7例,可3例,优良率达85.7%(18/21)。
结论:老年骨质疏松性桡骨远端骨折(AO-B、C型)有手术必要性,掌侧T-LCP内固定是有效的手术方法之一,具有固定可靠、退钉率低、早期恢复腕关节功能的优点。 |

| T-shape locking compression plate for distal radial fracture in aged patients |

ABSTRACT
Objective:To evaluate the clinical effects of T-shape locking compression plate(T-LCP)for distal radial fracture in aged patients(with osteoporosis)and to discuss the necessity of open reduction and internal fixation for these patients.
Methods:From January 2003 to March 2006,twenty-three patients with distal radial fracture were treated with fixation of T-LCP,7 were female and 16 were male with an average of 66 years old(range from 60 to 78).In this group,all the fractures were more severe than type B according to AO classification.The clinical effects were analyzed according to function of wrist joint and X-ray imaging after operation.
Results:Twenty-one cases were followed up from 6 to 18 months with an average of 9 months.Bone healing occurred in all the cases.The mean of angle of inclination in the palmar and ulnaris were respectively 4.5°,and 19.2°.The length of radial malleolus was longer than ulnar malleolus(more than 0.85 cm).According to criteria of joint functions,11 cases obtained excellent result,7 good,3 fair,the rate of excellent and good was 85.7%(18/21).
Conclusion:Operation has necessity for distal radial fracture(type B and C according to AO classification)in aged patients(with osteoporosis),T-LCP is one of effective method,which has such advantages as reliable fixation,minor complication and early recovery for joint funation. |
