目的:探讨椎间盘镜下中空螺钉内固定治疗齿状突骨折的围手术期处理方法。
方法:采用椎间盘镜下中空螺钉内固定治疗齿状突骨折12例,男9例,女3例;年龄20～55岁,平均34.9岁。受伤原因:车祸伤6例,平地摔伤2例,高处摔伤3例,重物砸伤1例。围手术期处理包括术前明确骨折类型、骨折牵引复位方法、手术期间的监测等。
结果:12例患者平均手术切口1.62cm、手术时间70min、出血量20ml。术后平均随访10.5个月,骨折全部愈合,愈合时间平均15周,颈椎活动基本正常,旋转(90.2°±16.9°),屈伸(40.5°±10.3°),无并发症发生。
结论:椎间盘镜下中空螺钉内固定术治疗齿状突骨折创伤小、手术操作方便,正确的围手术期处理对临床效果具有重要的作用。 |

| Perioperative management in treating odontoid process fracture with hollow screw fixation under diskoscope |

ABSTRACT
Objective:To explore the perioperative management in treating odontoid process fracture with hollow screw fixation under diskoscope.
Methods:Twelve patients(9 males and 3 females,the age from 20 to 55 years with the mean of 34.9 years) with odontoid process fracture,injury cause involved 6 traffic accident,2 falling in flat,3 falling from high and 1 smashed by the heavy matter.All the patients were treated with hollow screw fixation under diskoscope.Perioperative management including ensure the fracture type,method of traction and reduction,monitoring during operation and so on.
Results:The average incision length was 1.62 cm,operation time was 70 min and blood loss was 20 ml.The 12 patients were followed up with the mean of 10.5 months and all the fractures union with the mean of 15 weeks.The postoperative cervical vertebral motion range was close to normal,the mean rotation degree was(90.2°±16.9°) and flexion-extension degree was(40.5°±10.3°) without any complication.
Conclusion:Treatment of odontoid process fracture with hollow screw fixation under diskoscope has less trauma,simple operative and the correct perioperative management will be important for clinical efficacy. |
